Application Instructions
This application contains 6 sections that can be navigated by using the navigation bar to be located near the top of your application form pages. All of the sections of this application must be completed to the best of your ability. The different sections do not have to be completed in order, just so that they are completed when we receive your letter of
commitment. Make sure to press the save button at the bottom of any page to which you modify or add information.

Please ensure that all items are completed in the online application and all required signature documentation has been faxed to us.
Your application will be submitted to a committee of reviewers when you have filled out all sections and we have received the “commitment letter” from you and your school district. At that time, we will send you a confirmation e-mail. We will review your materials and notify you by e-mail and/or letter about the final status of your application. |
